About The Position
Join Heritage Health Network (HHN), a mission-driven organization caring for Medi-Cal members across six counties in California. We are seeking a Housing Navigator to join our Enhanced Care Management (ECM) team. This role will help connect individuals experiencing housing insecurity to safe, stable housing solutions while addressing their whole-person health needs.
Requirements
- At least 2 years of relevant experience.
- Bilingual Required: English/Spanish
Responsibilities
- Work with Medi-Cal members to secure housing and connect them with community resources.
- Collaborate with HHN’s multidisciplinary team (RNs, social workers, therapists, and more) to deliver person-centered care.
- Build trusting relationships and empower members toward independence and long-term stability.
