About The Position

The Housing Coordinator works with clients who are in need of housing solutions by outreaching and networking within the community in order to find best options for our clients. This role is part of the Opportunity Knocks program, a full service partnership (FSP) treating adults struggling with severe mental health disorders along with a current/recent legal history and are at risk of being homeless or currently homeless.

Requirements

  • Bachelor's degree from an accredited university in human services or related field required
  • At least 2 years of experience working with adults with mental health issues or related field preferred
  • Proof of valid California driver’s license and auto insurance, as well as proof of education are required.
  • May be required to have knowledge of rehabilitation model
  • Excellent documentation and organization skills, interpersonal skills and good written and verbal communication skills
  • Intermediate level PC skills required

Responsibilities

  • Works with treatment teams to assess housing needs of clients
  • Disseminating housing information to treatment teams
  • Research and building relationships with local housing options and vendors
  • Meets with clients to discuss housing options, managing contributions, keeping stable housing, and issues within housing that arise
  • Problem solves and supports clients with housing vendors as concerns happen
  • Organizes housing invoices, payments, and check requests for finance
  • Maintains daily record of work activities completing daily service progress notes and billing forms per Company and government standards and in compliance with Short Doyle/Medi-Cal requirements related to case management
  • Consistently meets monthly productivity requirement per hours established
  • Develop and maintain professional working relationships with agencies and service providers
  • Maintain and ensures HIPAA, ethical standards and professional boundaries with all staff and clients
  • Ability to work independently and make sound decisions without close supervision
  • Transport clients to accommodate community relationships. Use of personal car or vehicle is required
  • Other duties as assigned or necessary to support your department/program, office and/or the company
