About The Position

The Housing Coordinator (case manager) provides comprehensive person-centered case management services to individuals and families experiencing housing instability for those who are enrolled in Medicaid-funded programs. The Case Manager will assist clients in securing and maintaining safe, affordable housing while connecting them to community-based resources and supportive services. This role integrates housing placement, tenancy support, and healthcare coordination to promote long-term housing stability and reduced hospital utilization. HC will manage a caseload of 20 to 21 housing clients on Staten Island.

Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, or related field (required)
  • Minimum 1–2 years of experience in case management or housing services
  • Knowledge of NYC housing systems, rental assistance programs, and public benefits
  • Strong organizational and documentation skills
  • Ability to work with diverse populations, including individuals experiencing homelessness, mental health challenges, or substance use disorders
  • Understanding of Medicaid populations and social determinants of health
  • Strong interpersonal and communication skills
  • Crisis intervention and problem-solving abilities
  • Ability to manage multiple cases efficiently
  • Trauma-informed, housing-first approach
  • Proficiency in Microsoft Office and case management databases
  • Cultural competence and client-centered engagement

Responsibilities

  • Conduct intake assessments and develop individualized service plans
  • Conduct comprehensive biopsychosocial and housing assessments
  • Assist clients with landlord/property managers coordination
  • Assist clients with housing applications (NYCHA, supportive housing, rental subsidies, CityFHEPS, etc.)
  • Provide crisis intervention and ongoing case management support and eviction prevention support
  • Connect clients to employment services, public benefits, mental health services, and other community resources
  • Maintain accurate and timely case notes and documentation
  • Monitor client progress and adjust service plans as needed
  • Participate in case conferences and team meetings
  • Support clients with lease compliance, budgeting, and independent living skills
  • Collaborate with medical and behavioral health providers under Health Home guidelines
  • Assist with Medicaid benefits and entitlement recertification
  • Ensure compliance with MRT standards, and housing program regulations
  • Participate in case conferences and quality improvement initiatives
