The Housing Case Manager is responsible for providing home-based case management services for individuals and families experiencing homelessness who are rehoused throughout a 7-county service area. The primary goal is to assist individuals and families experiencing homelessness in obtaining permanent housing as quickly as possible, while making necessary connections to ongoing support to ensure long-term housing stability. The Housing Case Manager supports clients in maintaining housing by providing targeted referrals to services, providing regular home visits, and ongoing follow-up on community linkages to increase resiliency and greater self-determination. This position is responsible for quickly moving clients from homelessness to housing, using short-term interventions and connections to ongoing supportive services. Housing case management functions performed include housing procurement, administering financial assistance, housing stability planning, connection to community resources, income maximization, healthcare connections, and housing problem solving. Case management is crucial to the success of participants ability to maintain housing. This position is funded by the HUD Rapid Rehousing Grant. One80 Place ends and prevents homelessness throughout South Carolina with offices located in Charleston and Columbia. This position will be based in Charleston.
