About The Position

The Case Manager ensures that Allies’ clients living with HIV/AIDS have access to necessary housing, food, medical, mental health, drug & alcohol, and legal services by engaging them in the service planning process, and providing education, counseling, and referrals which promotes optimal health and independent living. Additionally, this position will coordinate the permanent supportive housing program available through HUD (Housing and Urban Development). Allies provides medical and non-medical case management services to clients in Allegheny, Armstrong, Beaver, Butler, Cambria, Fayette, Greene, Indiana, Somerset, Washington, and Westmoreland counties. The position will work collaboratively with other Client Services staff, our Health Advocates, and a multitude of agencies.

Requirements

  • Demonstrated experience in interviewing clients to gather information about their backgrounds, needs, or progress in order to develop and manage service coordination plans
  • Demonstrated experience working with a multitude of agencies to coordinate referrals
  • Demonstrated experience assisting clients from diverse backgrounds and circumstances in obtaining and maintaining housing
  • Basic knowledge and understanding of AIDS/HIV
  • Basic knowledge of local resources, referrals, and service systems
  • Knowledge of HIPAA and Act 148 standards
  • Knowledge of customer service and conflict resolution
  • Knowledge of Harm Reduction principles
  • Experience working with Electronic Health Records Systems
  • Strong communication and writing skills
  • Ability to balance and prioritize patient and administrative responsibilities
  • A Bachelor’s Degree in social work or similar area is required
  • Current driver’s license, auto insurance, and access to a reliable personal vehicle
  • Current PA child abuse, PA criminal history, and Federal criminal history clearances are required within the first 30 days of employment

Responsibilities

  • Work with assigned clients to develop measurable goals and maintain an up-to-date service coordination plan
  • Assess, monitor, evaluate, and record client progress according to measurable goals described in service coordination plan
  • Assist clients in determining initial and ongoing eligibility for HUD assistance
  • Communicate and enforce standards and contracts with clients and landlords
  • Seek out and maintain relationships with landlords
  • Maintain data in HMIS (Homeless Management Information System)
  • Utilize techniques and principles of Harm Reduction, Motivational Interviewing, and Trauma Informed Care
  • Maintain certifications, documentation, and administrative paperwork in accordance with program policies and procedures
  • Participate in annual HUD Monitoring process
  • Accompany clients to medical, housing, and social service-related appointments, and complete home visits and housing inspections
  • Link client to appropriate resources, in accordance with goals identified in service coordination plan, to provide access to services, and reduce barriers to care
  • Collaborate and coordinate with other professionals to assess and advocate for client’s medical, housing, and psychosocial needs
  • Advocate for clients to resolve crises and process emergency funding assistance
  • Record all client interactions and services in electronic case management system
  • Provide feedback to improve client services
  • Treat all clients, community members, and coworkers with dignity, respect, and unconditional positive regard – regardless of criminal history, substance use, sexual or gender identity, ethnic or racial background, or immigration status
  • Participate in case conferences, HUD meetings, and Ryan White Collaboratives as appropriate
  • Seek out up-to-date information about Allies’ programs and services, as well as relevant local, regional and national health statistics that will inform and support our Mission
