About The Position

Under the supervision of the Senior Housing Supervisor (SHS), the Housing Case Manager (HCM) is responsible for coordinating care for resident services. This role involves providing education, support, and accountability to ensure residents receive necessary services and the program remains compliant with all contractual and regulatory obligations. The HCM will work directly with residents to develop and implement service plans, assist them in achieving goals, facilitate groups, teach essential life skills, make referrals to community services, help access benefits, advocate for residents, and assist them in meeting tenancy obligations.

Requirements

  • Associate's Degree in a human services field or related field preferred.
  • Two years of experience in Human Services or a related field preferred.
  • A combination of education and experience may be substituted at the discretion of the Executive Director.
  • Ability to use hands to manipulate, handle, feel, and control items or equipment, including motor vehicles.
  • Ability to stand, sit, walk, bend, stoop, kneel, and reach.
  • Ability to climb up or down stairs.
  • Ability to reach above or below shoulders.
  • Ability to occasionally lift or move objects weighing up to 10 lbs.
  • Ability to sit at a desk or in a vehicle for long periods of time.
  • Ability to read, write, and interpret written documents.
  • Adherence to all applicable federal and state laws and regulations, including those governing confidentiality, privacy, program, billing, and documentation standards.
  • Successful completion of a background check.

Responsibilities

  • Meets regularly with the Senior Housing Supervisor (SHS) for a review of file documentation, ensuring compliance with funding regulations, agency policy, procedure, or business practice.
  • Participates in agency orientation of program operations, policy, procedure, goals and objectives.
  • Facilitates the integration of residents into the community, utilizing community-based services.
  • Maintains accurate case file records and reports based on the requirements of each funding source/program.
  • Responsible for the documentation, development, and/or implementation of assessments, service plan goals, service plan reviews, utilization reviews, hospitalizations, discharge planning, progress notes, and income in accordance with each residential housing program, ensuring compliance with agency policy, procedure, or business practice.
  • Submits resident files to the SHS within 28 days of admission for a complete file audit and review, ensuring compliance with program regulations, policy, procedure, or business practice.
  • Responsible for the renewal of file documents with expiration dates, including Release of Information (ROI) and/or Consents to Release Information, Physician Authorizations, Housing Quality Inspection packets, and Tenant Calculation Worksheets, Income Verification.
  • Ensures that licensed apartments are maintained at standards established for apartment recertification, reporting the need for household furniture/supply replacement or property maintenance.
  • Ensures resident fees related to rent and phone bills are paid in a timely manner and in accordance with agency policy, procedure, or business practice, assisting with delinquency issues as appropriate.
  • Assists residents in complying with treatment/service providers, ensuring no lapse in service/funding.
  • Serves as an advocate/mediator for residents experiencing difficulties with landlord/tenant disputes or provider, employment, funding related issues.
  • Acts as a resource and assists residents transitioning from the program to a more/less restrictive level of care, ensuring continuity of care with providers and funding streams.
  • Serves as a resource to ensure residents have access to services and that their rights and confidentiality are protected.
  • Conducts routine housing inspections to assess resident well-being and identify needs, communicating concerns with the SHS and working with residents to maintain their unit or access services.
  • Attends staff meetings, treatment team/provider meetings, and staff development/training seminars.
  • Conducts housing-related screening, client intake, and assessments to evaluate service/support needs and ensure documentation completeness.
  • Ensures all program criteria and eligibility are met in accordance with established regulations/guidelines/procedures.
  • Assesses, advocates, and provides linkages to community-based services.
  • Processes Catholic Charities residential housing admissions in accordance with established procedures.
  • Develops relationships with area landlords to facilitate recipient placement.
  • Assists recipients with the housing search process, including identifying suitable housing, accompanying them to view units, advocating with landlords, and assisting with paperwork.
