About The Position

Unity House of Troy is seeking a proactive, compassionate, and dedicated Housing Case Manager to join our Domestic and Sexual Violence Program. This role is ideal for someone committed to supporting survivors through trauma-informed, client-centered care while helping them achieve safety and housing stability. The Housing Case Manager provides advocacy and comprehensive case management services to individuals impacted by domestic violence. This position plays a critical role in supporting survivors through safety planning, housing navigation, and ongoing emotional and practical support.

Requirements

  • Bachelor’s degree in Human Services or related field (preferred) with 1 year of experience
  • Associate’s degree with 2 years of relevant experience (preferred)
  • High School Diploma/GED with 3 years of relevant experience may be considered
  • Valid, clean New York State Driver’s License and reliable transportation required

Nice To Haves

  • Experience working with survivors of domestic violence or similar populations strongly preferred
  • Knowledge of housing systems and community resources is a plus

Responsibilities

  • Provide victim-centered, trauma-informed case management services
  • Conduct safety planning, needs assessments, and individualized service planning
  • Advocate on behalf of clients to access housing and community resources
  • Maintain regular, supportive contact with clients
  • Assist clients in navigating housing options, applications, and requirements
  • Provide referrals and coordinate with community partners
  • Complete timely and accurate documentation, reports, and compliance activities
  • Promote a culture of inclusion, respect, and belonging
  • Represent and advocate for the program and the individuals we serve
