Requirements

Associate’s degree in Human Services or related field required, or 3 years direct experience in case management or related services required.
Ability to travel throughout the tri-county area regularly, even in inclement weather, using personal vehicle required.
Valid driver’s license.
Proof of current auto insurance.
Access to reliable transportation required.
Satisfactory criminal background check required.
Complete the Bloodborne Pathogens and Naloxone Training within 90 days of hire.
First Aid/CPR/AED certification within 90 days of hire.

Nice To Haves

Two years’ experience (i.e. employment, internships, and long-term volunteer position) in the field preferred.
English/Spanish bilingual preferred.
Bachelor’s degree in human services or related field preferred.

Responsibilities

Providing personalized case management and support to clients, ensuring their self-sufficiency and stable housing.
Compassionately and professionally conducting screenings and assessments to determine program eligibility.
Connecting clients with community resources.
Advocating for housing opportunities.
Maintaining precise client documentation.
Collaborating closely with the Client Services Manager to facilitate client success.
