About The Position

The Household Coordinator supports the culture of the Transitional Care Unit (TCU) at Aurora on France TCU, an Ebenezer community in Edina, Minnesota. The role is part of the subacute admissions care team and focuses on supporting patients, coordinating admissions and discharge planning, advocating for patients, and helping ensure a safe transition back to the community. The facility is connected to M Health Fairview Southdale Hospital and serves adults and seniors.

Requirements

• 0–1 years of experience.
• Proficiency in Microsoft applications.

Nice To Haves

• Bachelor’s degree in Social Work.
• 1–3 years of experience.

Responsibilities

• Positively support the culture of the household (TCU unit) by leading with a solution-focused mindset.
• Manage a caseload of 21 TCU patients.
• Facilitate smooth admissions into the facility for new patients.
• Complete intakes for up to 8 new admissions a day.
• Meet with new patients on the day they are admitted to begin discharge planning.
• Facilitate positive and safe discharges.
• Order discharge equipment, such as wheelchairs, walkers, hospital beds, and adaptive equipment.
• Arrange services for safe discharges, such as home care, wound care, oxygen, and IV infusions, as appropriate for each patient.
• Identify, investigate, and resolve complaints and grievances on the unit.
• Serve as the patient’s advocate and oversee patient quality of life in the facility.
• Create an exceptional experience that prioritizes safety and service to improve the experience, health, and wellbeing of everyone.
