About The Position
Full-time NP provides in-home primary care to medically complex patients. Be part of a comprehensive care team that includes skilled nursing, therapy, social work, caregiver and dementia care navigator.
Requirements
- Active Virginia NP license
- Medicare certification or willingness to obtain
- DEA registration or eligibility
- Experience in primary care, geriatrics and home-based care
Responsibilities
- Chronic disease management
- Hospital-to-Home Transition Care Management
- Advanced Care Planning
- Cognitive screenings
- Coordinate care with specialists, home health, hospice and personal care
- Educate patient and family on disease and treatment plan
