About The Position

The facility offers a broad range of clinical services including urgent care, primary care, geriatrics, chronic disease management, behavioral health, and home-based care services. The care model emphasizes collaboration, continuity, and timely access to care for patients with both acute and ongoing health needs. The facility is seeking a Nurse Practitioner – House Call to join its expanding care team in Brooklyn, New York. This is a full-time opportunity. The role focuses on delivering high-quality, patient-centered care by conducting in-home medical visits for patients who may have limited access to traditional clinic settings.

Requirements

  • Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
  • Board Certification as a Nurse Practitioner (FNP, ANP, or AGNP) through ANCC or AANP.
  • Active or eligible Nurse Practitioner license in New York.
  • Active Registered Nurse (RN) license in New York.
  • DEA registration (or eligibility).

Nice To Haves

  • Experience in primary care, geriatrics, home health, or internal medicine preferred.
  • New graduates with strong clinical training are encouraged to apply.

Responsibilities

  • Provide comprehensive in-home medical assessments including patient history and physical examinations.
  • Diagnose and manage acute and chronic medical conditions.
  • Develop and implement individualized treatment plans based on patient needs.
  • Order and interpret diagnostic tests including laboratory work and other relevant studies.
  • Provide medication management and monitor patient response to treatment.
  • Educate patients and caregivers on disease management, preventive care, and medication adherence.
  • Coordinate care with physicians, specialists, home health agencies, and family members.
  • Document patient encounters accurately and promptly within the electronic health record (EHR).
  • Participate in care coordination and quality improvement initiatives.
