About The Position

LSI is seeking passionate individuals to become Host Home Providers in the Eastern Iowa region. This role involves opening your home to an adult with disabilities, including intellectual, physical, mental health, and brain injuries. As a Host Home Provider, you will offer person-centered care, focusing on the individual's goals and support needs. This includes guiding and supporting them in daily activities such as shopping, budgeting, cooking, personal hygiene, and community integration. You will act as an advocate and mentor, helping the individual live their best life and become a part of your family. This is a contracted position where you serve as an independent provider. LSI provides matching services, support, guidance, and resources for training. A home safety check and training are required before an individual can be placed.

Requirements

Must be 21 or older
Must reside in Iowa
Must have a high school diploma or GED
Must have a current/valid driver's license
Must meet minimum insurance requirements
Must have access to a computer/internet to complete documentation requirements
Must successfully complete all steps of the extensive screening, interview, and matching process before an individual can begin receiving support in your home
Must have flexibility and creativity
Must have a passion for making a difference in the world
Requires a valid driver's license, auto insurance, commercial general liability insurance, a decent driving record
Ability to pass an extensive background check including checks of the Dependent Adult and Child Abuse registries, Sex Offender registry, criminal history check, reference checks and a home safety check.
Background checks are also required of all the people who reside in the home who are over the age of 18.

Responsibilities

Provide guidance, support, and care for the individual living in your home.
Work with the mentee on goals such as shopping, budgeting, coping skills, cooking meals, and personal hygiene.
Assist the mentee in integrating into their community.
Act as an advocate/mentor helping them to live their best life possible.
Invite the individual to become part of your family.
