Hospitalist Per Diem Advanced Practitioner
About The Position
Ellis Medicine is seeking a Per Diem Hospital Medicine Advanced Practitioner to join our collaborative Hospital Medicine team. This is an excellent opportunity for an experienced APP or new graduate seeking flexible clinical work within a well-supported Hospital Medicine program. Our APPs play an integral role in the care of hospitalized patients, working collaboratively with physicians while managing a dedicated patient census. With strong physician support, broad specialty availability, a closed ICU and no required procedures, our APPs can focus on providing high-quality, patient-centered care.
Requirements
- Current NP or PA certification
- Licensed (or eligible) to practice in New York State
- Strong clinical judgment and communication skills
- Ability to work collaboratively within a multidisciplinary team
- Commitment to high-quality, patient-centered care
Nice To Haves
- Hospital medicine or inpatient experience preferred but not required
Responsibilities
- Provide comprehensive inpatient care in collaboration with Hospital Medicine physicians
- Evaluate and manage hospitalized patients
- Perform daily rounds and develop individualized treatment plans
- Complete admission and discharge evaluations as assigned
- Coordinate care with physicians, nursing staff and consulting specialists
- Communicate treatment plans and clinical updates with patients and families
- Participate in multidisciplinary care planning
- Coordinate transitions of care and follow-up needs
- Provide evidence-based, patient-centered care
