About The Position

As a Primary Care Advanced Practice Provider (APRN), the role delivers comprehensive, patient-centered care across the continuum in collaboration with physicians and an interdisciplinary care team. The provider manages acute, chronic, and palliative care needs; conducts comprehensive assessments; develops individualized care plans; diagnoses and treats conditions; interprets diagnostic results; prescribes medications; and coordinates services to support patient outcomes. The provider practices at the top of their license and supports quality outcomes through care gap closure, HCC documentation, chronic disease management, and preventive care.
