Requirements

Graduate from accredited Nurse Practitioner program with MSN or DNP
Nurse Practitioner license and furnishing license
Board Certification
New Nurse Practitioner graduates will be considered with Adult Gerontology Acute Care Board Certification and RN inpatient bedside experience

Two year experience preferred as an Nurse Practitioner

Responsibilities

Judiciously manage resources to ensure hospitalization is efficient, expedited and evidence based.
Develop treatment plans, prescribe medications, order and interpret diagnostic tests, engage with consultants and deliver comprehensive documentation.
Educate patients and families to take full advantage of all resources that will support and enhance clinical outcomes during and after hospitalization.
Collaborate with a dedicated team of both inpatient and outpatient case managers for seamless discharge planning and outpatient services to reduce the risk of readmission.
