About The Position

The Hospitalist APP will be responsible for providing medical services to hospitalized patients. This role involves following up on patient care, coordinating with various providers, communicating with patient families, and managing unassigned medical patients from the Emergency Department. The APP will conduct daily rounds, complete necessary documentation for billing, and perform history and physical examinations within specified timeframes.

Requirements

NYS Licensed/Registered NP or PA
Ability to hold and maintain Community Memorial Hospital active medical staff membership and clinical privileges appropriate to duties and responsibilities.
Current, valid and unrestricted federal and state registrations to prescribe and dispense controlled substances.

Responsibilities

Work 12-hour shifts for 7 consecutive days, followed by 7 consecutive days off.
Cultivate a work climate supportive of the Hospital’s mission, vision, and values.
Commit to a customer service-oriented culture.
Follow up on patient care, discuss patient care with nurses, and resolve problems.
See patients more than once a day as needed.
Coordinate care between different providers caring for a patient.
Serve as a point of contact for hospital staff.
Maintain communication with the patient’s family.
Accept and manage unassigned medical patients from the Emergency Department or other departments requiring inpatient admission.
Provide professional medical services to hospitalized patients.
Arrange for necessary follow-up care upon discharge.
Make daily rounds on all patients assigned to the Hospitalist Program.
Complete all necessary documentation for the Hospital to bill and collect for medical services rendered.
Conduct a history and physical examination within twelve (12) hours of patient assignment.
Have the history and physical note dictated within 24 hours of admission.
Perform other duties as assigned.
