About The Position

Performs activities supporting the Case Management/Utilization Management team and promotes optimal health care outcomes in accordance with Community First’s policies, procedures, applicable laws and contracts, philosophy, objectives, and mission. The position also supports the Behavioral Health Unit. This is a full-time position with hours of 8:00 a.m. to 4:30 p.m.

Requirements

LSW required; LCSW preferred.
At least 2 years of clinical social work experience required.
Bachelor’s degree required.
Behavioral experience preferred.
Acute-care social work experience preferred.
EMR/EPIC experience preferred.
Ability to perform the essential duties satisfactorily; reasonable accommodations may be made for individuals with disabilities.

Responsibilities

Identify alternative practice among physicians, nurses, and other health care professionals.
Evaluate and document patients’ clinical status, post-hospital needs, and plans.
Monitor care delivery and patients’ responses to care during hospitalization.
Assess patients’ physical status to revise the plan of care.
Discuss treatment plans with patients, families, and/or significant others.
Communicate changes in patients’ status and plans of care to involved staff members.
Educate medical staff and other team members about proper documentation of services rendered.
Screen cases for risk management, infection control, or quality-of-care issues.
Identify discharge-planning needs and anticipated discharge outcomes.
Perform admission, concurrent, and discharge reviews.
Communicate and provide updated clinical and medical information and anticipated discharge dates.
