About The Position

The Hospital Liaison is responsible for conducting screens and assessments for newly delivered mothers and newborns to determine eligibility status for Health Beginnings Entry Agency referral services. The Hospital Liaison effectively promotes program goals and communicates accurate information about the Healthy Beginnings program. This position will provide services in assigned hospital location(s) and may require scheduling flexibility in order to accommodate hospital schedules.

Requirements

Bachelor’s degree in Human Services (preferably in a field such as social work, developmental psychology, and/or early childhood education).
Minimum of two (2) years’ experience in child welfare, primary prevention services, child development or related field.
Schedule includes occasional evening and weekend hours as needed
Bilingual English/Spanish required.
Valid Florida’s driver’s license is required.

Responsibilities

Conduct screening assessments with new mothers and infants in hospital settings to determine eligibility for support services.
Partner with hospital staff to ensure accurate referrals, smooth communication, and effective program coordination.
Identify family strengths, risk factors, and service needs using approved assessment tools.
Connect eligible families to community resources, support services, and program referrals that promote family well-being.
Maintain timely documentation, data entry, and compliance with program standards, outcomes, and reporting requirements.
