About The Position

Join a fast-paced remote team supporting vulnerable populations through compassionate, trauma-informed care. In this role, you’ll manage incoming referral calls, coordinate admissions into available inpatient beds, and connect individuals to appropriate levels of care when inpatient placement is not suitable. Responsibilities include communicating with referral partners, guardians, and individuals in crisis, de-escalating challenging situations, multitasking across multiple systems and EHR platforms, and collaborating with internal and external teams to ensure timely placements. Ideal candidates are organized, professional under pressure, strong communicators, and passionate about making a positive impact through service and support.

Requirements

  • Bachelor’s degree in a relevant field (i.e. Social Work, Education, Sociology, Psychology, Counseling, Human Relations, or Criminal Justice) preferred.
  • A high school diploma or equivalent with a minimum of two years of relevant experience (e.g., working with vulnerable populations, intake coordination, or crisis management may be considered.
  • Valid drivers license and auto insurance.
  • At least 21 years of age.
  • One or more years of related experience.

Nice To Haves

  • Supervisory experience preferred.
  • At least two years of experience working with economically disadvantaged, vulnerable, or at-risk youth and adults.
  • Highly organized, technologically proficient, and comfortable working in a fast-paced remote environment using Microsoft 365, Electronic Health Records (EHR), OneDrive, Teams, and other digital tools.
  • Strong multitasking, communication, and time management skills are essential for handling calls, emails, documentation, and care coordination simultaneously.
  • Able to work independently, troubleshoot problems effectively, and collaborate with remote teams while maintaining accuracy and attention to detail.
  • Experience with crisis intervention, de-escalation techniques, and trauma-informed care is strongly preferred.

Responsibilities

  • Provide respectful, trauma-informed support to children, youth, adults, families, and community partners
  • Manage intake calls, walk-ins, and referrals for psychiatric hospital admissions
  • Coordinate placements into available inpatient beds or refer individuals to appropriate outpatient care
  • Communicate with referral partners, guardians, health plans, and internal teams to ensure timely care coordination
  • De-escalate crisis situations using approved trauma-informed techniques
  • Complete accurate intake assessments and maintain detailed documentation in Electronic Health Records (EHR) systems
  • Maintain confidentiality and ensure accurate data entry, reporting, and recordkeeping
  • Multitask across multiple systems and communication platforms, including EHR and Microsoft Teams
  • Work independently to troubleshoot issues and maintain efficient workflow in a fast-paced remote environment
  • Build positive relationships through strong communication, active listening, and interpersonal skills
  • Stay current on organizational policies, procedures, programs, and community resources
  • Maintain organized, professional workspaces and support daily operational needs
  • Participate in staff meetings, trainings, and ongoing professional development opportunities
  • Demonstrate professionalism, attention to detail, and commitment to diversity, equity, and inclusion
