The Hospice Triage Registered Nurse (RN) receives incoming triage calls from patient and providers, addressing changes in physician orders and patient care needs as appropriate. Requirements
- Associate's Degree Accredited Registered Nurse Program - Required
- Registered Nurse (RN) - State Licensing Board - Required
- Cardiopulmonary Resuscitation (CPR) - Required
- Auto Insurance - Required
- Driver's License - Required
Nice To Haves
- 1 year - Hospice Experience - Preferred
Responsibilities
- Assist patients, families, clinical staff and providers via telephone with clinical issues, problems and questions.
- Report changes in patient status to appropriate clinician.
- Participate in case conferences and interdisciplinary team meetings and coordinate changes in physician orders with clinical staff and providers.
- May provide nursing visits as required to deliver patient care.
- Receive physician orders, provide follow-up with appropriate clinical staff; may include scheduling.
- Complete required documentation within the established timeframe.
