About The Position

Lightways Hospice and Serious Illness Care, an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois, is seeking a full-time Hospice Team Leader for DuPage County. The Hospice Team Leader will oversee a team of RN Case Managers, Social Workers, and Chaplains, planning, organizing, and directing hospice care. This role requires experience in nursing, with an emphasis on community health education/experience. The majority of direct patient care is provided in the field, with some staff based in Joliet administrative offices and a 24-bed inpatient unit in Joliet.

Requirements

RN with a minimum of 3 years Hospice experience.

Nice To Haves

Prior supervisory experience a plus.

Responsibilities

Enhance communication to ensure existing RN Case Managers, Social Workers and Chaplains receive support in the field.
Conduct weekly IDT meetings for their designated team.
Guide new staff with new employee orientation and education in hospice care.
Perform timely performance check-ins with their team on a quarterly basis.
Participate in ride-a-long visits with their team on a quarterly basis and as needed.
Maintain a small caseload.
Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
Participate in on call coverage.
