As our Social Services Team Lead, you'll combine clinical expertise with leadership responsibilities to ensure exceptional psychosocial care throughout our hospice program.

Requirements

  • Master's Degree in Social Work (MSW) from a CSWE-accredited program.
  • Current, active Arizona social work license in good standing: LMSW or LCSW accepted.
  • Minimum of three (3) years of professional social work experience in hospice, palliative care, healthcare, home health, or a related clinical setting.
  • Minimum of one (1) year of leadership, supervisory, or management experience.
  • Strong understanding of psychosocial assessment, family dynamics, and end-of-life care.
  • Valid Arizona driver's license, reliable transportation, and current automobile insurance.

Nice To Haves

  • Hospice experience strongly preferred.
  • Previous experience supervising social workers.
  • Knowledge of Medicare hospice regulations and interdisciplinary team practices.
  • Bilingual candidates are encouraged to apply.

Responsibilities

  • Supervise, mentor, and support a team of Licensed Social Workers.
  • Foster a positive, collaborative, and patient-focused work environment.
  • Assist with onboarding, training, coaching, and professional development.
  • Conduct performance reviews and provide ongoing guidance and support.
  • Serve as a clinical resource for complex patient and family situations.
  • Promote best practices and high standards of care across the social work team.
  • Oversee psychosocial assessments and care planning activities.
  • Collaborate with nurses, physicians, chaplains, aides, and other interdisciplinary team members.
  • Support patients and families facing emotional, financial, social, and end-of-life challenges.
  • Assist with crisis intervention, counseling, resource coordination, and advance care planning.
  • Ensure services are delivered with empathy, respect, and cultural sensitivity.
  • Help ensure compliance with Medicare Conditions of Participation, Arizona regulations, HIPAA requirements, and hospice industry standards.
  • Monitor documentation quality and regulatory compliance.
  • Participate in quality improvement initiatives, audits, and survey readiness activities.
  • Support operational excellence while maintaining exceptional patient-centered care.
