Hospice Staff RN/Case Manager - (0.6 FTE)
About The Position
The Hospice Staff RN/Case Manager provides nursing care to hospice patients in coordination with the interdisciplinary team (IDG) that is consistent with the goals, definitions, philosophy, and purpose of HIA Hospice. This includes all nursing care approved by patients’ primary physicians or the hospice medical director, such as hospice referrals, patient admissions, physical assessments, symptom management, and emotional support. This position is part-time (0.6 FTE - 24 hours per week), benefit eligible and will be based out of our Jamestown office and serve the surrounding communities.
Requirements
- Minimum of RN degree
- Minimum of one year of experience as a hospital staff nurse and/or minimum of two years of experience in health care
- Strong interpersonal communication, counseling, and teaching skills
- Flexibility in scheduling; team player
- Current state RN licensure in North Dakota and Minnesota
- Current driver's license and auto insurance
Nice To Haves
- BSN preferred
- Familiarity with principles related to dying and death strongly preferred
