As a Registered Nurse Case Manager, you’ll contribute to our success in the following ways: Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs. Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team. Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care. Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings. Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

Requirements

  • Current State license as a Registered Nurse.
  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
  • 1+ year of experience as a nurse in a clinical care setting.
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

  • Experience in hospice or home health environment is preferred
