About The Position

Responsible for the provision of spiritual care services to patients/families/caregivers of the hospice program directly or through coordination of care with community spiritual counselors. Cares for neonate, infant, child, adolescent, adults and geriatric clients. Performs clinical duties in accordance with population specific guidelines and applicable regulatory standards.

Requirements

  • Current BLS or obtain BLS within 30 days of employment.
  • Masters Degree.
  • Ordination or commission by a recognized faith group and possesses at least one unit of clinical pastoral education from a nationally recognized provider.
  • Experience in pastoral counseling.
  • Experience with death and dying with individuals and families.
  • Communication, organization, pastoral/spiritual care counseling skills
  • Demonstrated ability to be open, sensitive, flexible and ecumenical, and to work as a member of an interdisciplinary team
  • Computer skills

Responsibilities

  • Provides spiritual assessment, counseling and support to patients and families in the patient's place of residence as indicated.
  • Participates in the preparation, evaluation and execution of the interdisciplinary care plan to assure that the aims of continuity and comprehensiveness of care are accomplished.
  • Participates in the Interdisciplinary Team meetings, staff meetings, and other meetings as requested.
  • Works collaboratively as a member of Interdisciplinary Team.
  • Facilitates linkage to community clergy as desired by patients/families.
  • Participates in agency and community programs, including supervision, to enhance learning and performance.
  • Participates in professional liaisons with other clergy and pastoral and healthcare providers.
  • Demonstrates and applies knowledge of diverse religious, cultural and spiritual values and customs related to illness, death and mourning.
  • Provides spiritual care in a non-judgmental manner that honors the unique spiritual needs of the patient/family.
  • Participates in taking on-call responsibilities as necessary including weekends, holidays and evenings.
  • Participates in providing support to staff and volunteers in coping with job-related stress.
  • Provides consultation, orientation, education and support in spiritual care to members of the Interdisciplinary Team.
  • Coordinates spiritual care component of the hospice service.
  • Maintains records of spiritual care services utilization and related activities for quality assurance and program development.
  • Participates in Policy and Procedure development and revision.
  • Participates in the recruitment, training, supervision and periodic evaluation of volunteer chaplains.
  • Participates in clinical and community outreach activities such as community education and patient consultation/admissions.
  • Performs memorial and funeral services for hospice program, clients and contracted facilities as requested.
  • Performs other duties as requested.
