About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring a Spiritual Care Counselor for a Hospice position providing care to patients in their homes in the Troy area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. The counselor will conduct spiritual assessments, develop care plans, provide spiritual counseling and emotional support to patients and families, address complex needs, advocate for patient wishes, and maintain EMR documentation. The role requires independent work with a supportive team structure and travel to patient homes within an assigned territory.

Requirements

• Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred.
• Experience working with death and dying, individuals/family/caregiver.
• Hospice experience preferred.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Conduct spiritual assessments to identify patient and family needs, helping develop care plans that support emotional, spiritual, and end-of-life goals.
• Provide spiritual counseling, emotional support, and guidance to patients and families of diverse backgrounds and beliefs, helping them navigate the end-of-life journey with comfort, dignity, and compassion.
• Address complex emotional and spiritual needs, advocate for patient wishes, and maintain accurate, timely, and compliant EMR documentation.
• Work independently but never alone.
• Travel to patient homes within assigned territory.
