About The Position
Moments Hospice is hiring a Licensed Social Worker to provide psychosocial support to terminally ill patients and their families. The role includes assessing patient and family needs, implementing care plans, providing counseling and referrals, and supporting patients and families through terminal illness, pre-bereavement, and bereavement. The position offers autonomy, reasonable caseloads, on-call support, structured training, and collaboration with company leadership. It is listed as PRN and on the road.
Requirements
- Current Social Worker license.
- At least two years of social work experience in a healthcare setting or social services agency.
- Bachelor’s degree in Social Work with at least one year of healthcare experience; a Master’s degree in Social Work is preferred.
- Knowledge of and ability to obtain community resources, including information and referral services and state and federal financial programs and eligibility requirements.
- Understanding of hospice philosophy and the principles of death and dying.
- Strong organizational and communication skills.
- Valid driver’s license.
- Valid auto insurance.

Responsibilities
- Provide a wide range of psychosocial services to terminally ill patients and their families.
- Contact the patient, family, and caregiver within 48 hours of admission.
- Perform case manager duties.
- Ensure the psychosocial assessment is added to the patient’s chart within 24 hours of completion.
- Regularly assess the psychosocial needs of patients and families.
- Regularly assess pre-bereavement and anticipatory grief needs.
- Implement the Plan of Care, including social service goals, supportive counseling, problem solving, community referrals, and pre-bereavement and bereavement care.
- Regularly re-evaluate the psychosocial needs of patients and families.
- Participate in weekly interdisciplinary team meetings.
- Assist with funeral arrangements when requested.
- Provide counseling during terminal illness and at the time of death when requested.
