About The Position
We have an immediate part-time opening for a temporary position in hospice social work! In this role, you'll provide support to hospice clients in the northern suburbs (Lake and McHenry counties).
Requirements
- LSW or LCSW
- Hospice/end-of-life care or home health experience
- Willingness to travel throughout Lake and McHenry counties to make home and facility visits to clients.
- Personal vehicle + valid drivers license and insurance
