About The Position

The Medical Social Worker PRN is responsible for providing clients and families with psychosocial care regarding emotional, social, and economic needs, according to prescribed policies and procedures of the organization, including all applicable federal, state, and accreditation regulations. This position operates under the general supervision of the Director of Clinical Management. The role does not have a set weekly schedule; duties and schedules are assigned as needed, consistent with part-time or contingent employment. Mileage reimbursement is provided.

Requirements

  • Master’s degree in social work from a school of social work accredited by the Council on Social Work Education.
  • Minimum of one year of social work experience in a health care setting.
  • Must have good communication, comprehension, computer and interpersonal skills.
  • Must have the ability to speak, read, write and understand English.
  • Must have good working computer skills with basic professional computer software (Microsoft Office suite, Windows, etc.) and the capacity for learning new software systems quickly.
  • Must be self-directed with excellent organizational skills and the ability to manage tasks and assignments concurrently.
  • Must have a current social worker license in good standing.

Nice To Haves

  • Two (2) years in varied multi-disciplinary settings in the health care field with experience working with clients and family counseling.

Responsibilities

  • Provides services ordered by the physician as indicated in the plan of care and carries out social evaluations and plans intervention based on evaluation findings.
  • Maintains clinical records on all clients referred to social work.
  • Provides information and referral services to organization clients and families/caregivers regarding practical and environmental needs.
  • Provides education to and serves as a liaison between clients or families/caregivers and community agencies.
  • Participates in the development of the total plan of care, including case conferences and discharge planning, as required.
  • Assists the physician and other team members in understanding significant social and emotional factors related to health problems, as needed.
  • Participates in the QAPI program and HHA sponsored in-service training, as required.
  • Participates in interdisciplinary team meetings to coordinate the care of the client and family; exchange information and problem-solve; and receive staff support and education.
