About The Position

At Lifted Hospice, our Social Workers play a vital role—providing emotional support, counseling, and practical guidance when patients and families are navigating one of life’s most difficult seasons. As a Social Worker, you’ll assess each patient’s psychosocial needs and deliver care that addresses emotional, financial, and family dynamics. You’ll help patients and caregivers understand their options, cope with change, and access the resources they need for comfort and stability. You’ll also serve as a key part of the interdisciplinary team—bringing insight, advocacy, and compassion to the plan of care. In this role, you’ll spend your time serving patients and families wherever they call home—whether in a private residence, long-term care facility, or nursing facility. Your work will take place in varied environments, both indoors and outdoors, as you meet people where they are in some of life’s most vulnerable moments. Flexibility, presence, and professionalism are essential as you travel locally to provide compassionate, timely care and support across your service area.
