Hospice Social Worker
About The Position
The Parker Jewish Institute for Health Care and Rehabilitation is seeking a full-time Social Worker to join the Hospice department. Parker Jewish Institute Hospice offers high-quality hospice services through an interdisciplinary team. The program provides personalized care to ensure comfort, enhance quality of life, and preserve the dignity of patients with advanced or life-limiting illnesses and their families. This role involves field visits in Queens and Nassau counties, as well as in-patient visits. The Hospice Social Worker will deliver appropriate social work services to assigned patients, residents, family members, and significant others. Responsibilities include performing initial psychosocial, emotional, spiritual, and bereavement assessments, ongoing assessments, and assisting in the development and implementation of goal-directed IDG care plans, while advocating for the patient and family.
Requirements
- Master's Degree in Social Work
- Active NYS Social Work License
- Prior experience as a Social Worker in a community health, hospice or inpatient setting
- Valid New York State driver's license
- Able to make field visits in Queens and Nassau counties
Responsibilities
