About The Position
Tapestry Hospice & Palliative Care is seeking an experienced Hospice Social Worker to provide emotional and practical support to patients and families facing serious illness or end-of-life issues. This position is part-time and works closely with members of the Interdisciplinary Team, patients, and their loved ones during the hospice experience. The role is one of three foundational pillars in the patient's overall plan of care, managing all psychosocial aspects of the hospice journey.
Requirements
- Licensed as a Social Worker through the Board of Georgia
- Possess effective communication skills (including strong verbal and written)
- Possesses a history of building and influencing patient and family relationships
- Holds a current driver's license, car insurance, CPR certification, and positive driving record
- Part-time availability
Nice To Haves
- Hospital, home health, or hospice experience is extremely preferred
Responsibilities
- Assess patient/family social and emotional factors to estimate their potential to cope with terminal illness and death.
- Provide psychosocial counseling and support to the patient/family experiencing emotional, social, and economic conflict.
- Participate in Interdisciplinary Team (IDT) meetings to establish, review, and revise the plan of care for each patient, including exchange of information, review of problems, and sharing of professional support.
- Maintain the dignity of the dying patient.
