About The Position

Care Initiatives is currently seeking a PRN Hospice Social Worker to provide patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities. This role involves providing comprehensive care to patients during their final stages, meeting them and their families where they feel most comfortable (home, long-term care community, assisted living residence, or hospital). The approach is holistic, addressing physical, emotional, and spiritual needs, and working collaboratively with family members and caregivers to honor patient wishes. The goal is to provide care with compassion, dignity, and respect during this significant phase of life.

Requirements

• Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education
• Current, valid social work license (LMSW)
• Current driver's license
• Automobile liability insurance
• Ability to travel between multiple locations within your region

Nice To Haves

• Hospice or palliative care experience
• Two (2) years of verifiable experience working as a Social Worker

Responsibilities

• Assess patients’ psychosocial needs and complete social histories
• Document care plans, progress, and follow-up per regulations
• Provide counseling and emotional support to residents and families
• Support patients and families with adjustment, rights, and orientation
• Coordinate referrals and act as a liaison with community resources
• Partner with the care team to enhance quality of life and outcomes
• Plan and support patient, family, and community engagement activities
• Promote facility programs and services to referral partners
