About The Position

Care Initiatives is currently seeking a PRN Hospice Social Worker to provide patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities. Our Social Workers go above and beyond by meeting patients and families where they feel most comfortable, whether it's at home, a long-term care community, assisted living residence, or hospital. This personalized approach ensures tailored care that meets their unique needs. Requirements

• Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education
• Current, valid social work license (LMSW)
• Current driver's license
• Automobile liability insurance
• Ability to travel between multiple locations within your region

Nice To Haves

• Hospice or palliative care experience
• Two (2) years of verifiable experience working as a Social Worker

Responsibilities

• Assess patients’ psychosocial needs and complete social histories
• Document care plans, progress, and follow-up per regulations
• Provide counseling and emotional support to residents and families
• Support patients and families with adjustment, rights, and orientation
• Coordinate referrals and act as a liaison with community resources
• Partner with the care team to enhance quality of life and outcomes
• Plan and support patient, family, and community engagement activities
• Promote facility programs and services to referral partners
