About The Position

Sanctuary Hospice is seeking a patient-centered Licensed Master Social Worker (LMSW) to provide social work services to hospice patients and their families. The role provides counseling and guidance to support emotional well-being and coping capacity, and helps bring dignity, peace, and human connection to patients and loved ones during end-of-life care. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m. The coverage area includes Northern Cook, Lake, and McHenry County. The position works with the RN Case Manager, interdisciplinary group, and Bereavement Coordinator to assess psychosocial needs, advocate for individualized care, and contribute to care planning.

Requirements

Master of Social Work from an accredited school of social work.
Licensed as a Social Worker by the State Board of Social Work.
Current driver’s license and vehicle insurance, with access to a dependable vehicle.
At least one year of experience as a social worker in a healthcare setting.

Nice To Haves

At least one year of experience as a social worker in a hospice setting.
Experience in a long-term care or medical-surgical/acute-care setting.

Responsibilities

Assess and communicate patients’ psychosocial status to the RN Case Manager and interdisciplinary group.
Evaluate medical needs, home situations, financial resources, and community support.
Implement personalized social work plans and collaborate with the Bereavement Coordinator.
Help shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
Provide empathetic counseling to patients and families.
