Iowa City Hospice is growing and seeking a compassionate and dedicated Part-Time Social Worker to join our team. As a Hospice Social Worker, you will be responsible for providing providing emotional support, counseling, and community resources to patients and their families from time of admission until end of services; then assessing the bereavement needs of the patient’s survivors. This position requires exceptional communication skills, strong attention to detail, adaptability, and the ability to successfully manage responsibilities in an ever-changing work environment. Candidates must demonstrate professionalism, initiative, and consistent follow-through while working collaboratively with patients, families, community partners, and healthcare professionals. Key responsibilities include conducting comprehensive psychosocial assessments to identify the emotional, social, financial, and support needs of patients and their families; developing and implementing individualized care plans; providing counseling, guidance, and emotional support to help patients and families cope with illness and end-of-life challenges; and connecting individuals with appropriate community resources, support services, and support groups. The role also involves advocating for patients and families, monitoring and evaluating patient progress, maintaining accurate and confidential documentation, and collaborating with interdisciplinary healthcare teams to ensure comprehensive, patient-centered care. Participation in team meetings and ongoing care planning discussions is expected to support quality outcomes and continuity of care.
