About The Position
As a Part-Time Hospice Social Worker with Cardinal Hospice, you will have the opportunity to build meaningful relationships, support families, advocate for patients, and provide compassionate care when it matters most. Requirements
- A degree in Social Work or a related field is required.
- Appropriate social work licensure or credentials as required.
- Strong communication, organizational, and interpersonal skills.
- Compassionate, dependable, and able to work independently as part of an interdisciplinary team.
- Ability to travel to patient locations as needed.
Nice To Haves
- A Master’s degree is preferred.
- Hospice, healthcare, or social work experience preferred.
Responsibilities
- Provide emotional, psychosocial, and practical support to hospice patients and families.
- Assess patient and family needs and develop appropriate care plans.
- Provide counseling, crisis intervention, and grief support as needed.
- Connect patients and families with community resources and services.
- Advocate for patient and family needs, dignity, and quality of life.
- Collaborate with nurses, physicians, chaplains, aides, and other members of the hospice team.
- Maintain accurate and confidential documentation in compliance with HIPAA and hospice standards.
