Hospice Social Worker, MSW
About The Position
Spirit Hospice is seeking a licensed Social Worker to join their team. This role is an equal pillar to nurses and chaplains, requiring someone who can support patients and families through difficult times with grace, strength, and compassion. The Social Worker will help families navigate grief, fear, and paperwork, while advocating for the patient's dignity and reminding them of the love that remains even in loss. Collaboration with an interdisciplinary team, including nurses, chaplains, aides, doctors, and volunteers, is essential for developing and executing patient-specific care plans. The role involves offering resources, counseling, and a grounding presence during a challenging period.
Requirements
- A current Social Work License in the state of Georgia.
- Hospice, medical, or mental health experience. (and if not hospice yet, a calling to it)
- A steady heart that can hold both tears and laughter in the same breath.
- Strong communication, organizational, and advocacy skills.
Responsibilities
- Walk alongside patients and families through some of life’s hardest but most meaningful moments.
- Help families navigate the messy parts.. grief, fear, and paperwork.
- Lead with compassion, advocate fiercely, and remind people that even in loss there’s still love and dignity.
- Collaborate closely with nurses, chaplains, aides, doctors and volunteers in our interdisciplinary team (IDT), ensuring care is heart-centered.
- Develop and execute patient-specific care plans
- Offer resources, counseling, and that grounding presence people remember long after the goodbye.
