Requirements

Master's degree in Social Work (MSW) from an accredited institution.
Current state licensure as a Social Worker (LCSW or equivalent).
Minimum of 1-2 years of experience in hospice or palliative care.
Strong understanding of psychosocial needs of terminally ill patients and their families.
Excellent communication, interpersonal, and counseling skills.
Proficiency in electronic health record (EHR) systems.
Ability to work independently and as part of an interdisciplinary team.
Valid driver's license and reliable transportation.

Nice To Haves

Experience with grief and bereavement counseling.
Knowledge of community resources and referral processes.
Bilingual abilities (e.g., Spanish).

Responsibilities

Provide psychosocial assessments and support to patients and families.
Develop and implement social work care plans.
Coordinate with the interdisciplinary team to ensure comprehensive patient care.
Provide counseling and crisis intervention as needed.
Facilitate family meetings and support groups.
Assist with discharge planning and connection to community resources.
Maintain accurate and timely documentation of all services provided.
