About The Position

ANX Hospice Care provides 24/7 hospice care for individuals with limited life expectancies or those no longer seeking aggressive medical treatment. Their specially-trained team includes a medical director/physician, medical social worker, skilled nurse, home health aide, spiritual counselor, and hospice volunteer. Requirements

  • High school diploma or equivalent required
  • 1-2 years of experience in healthcare scheduling or a related field.
  • Excellent communication and organizational skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Compassionate and empathetic approach to working with patients and families.
  • Proficiency in electronic medical records (EMRs) and scheduling software.
  • Familiar with the geographical location of different counties we serve: San Francisco, San Mateo, Alameda and Contra Costa.

Nice To Haves

  • associate's or bachelor's degree in a related field preferred.
  • Experience working in hospice or palliative care.
  • Certification in medical scheduling or a related field.
  • Knowledge of healthcare regulations and compliance.

Responsibilities

  • Schedule patient admissions with Admission nurse; F2F visits with Nurse Practitioner or Hospice Physician
  • Manage and coordinate schedules for interdisciplinary team visits (IDG), including physicians, nurses, social workers, spiritual care providers and ancillary staff.
  • Communicate effectively with patients, families, and healthcare professionals to ensure smooth transitions and continuity of care.
  • Maintain accurate and up-to-date records of patient schedules, visits, and services.
  • Collaborate with the admissions/intake team to ensure timely and efficient patient intake processes.
  • Troubleshoot scheduling conflicts and resolve issues promptly.
  • Provide administrative support to the admissions/intake team as needed.
  • Be on top of all PTO or time off request by staff and inform team
  • Inform facilities, families of staff schedules, any changes via email, calls or faxes or a combination of.
  • Assist with plotting orientation schedule for new hires, any scheduled CPR classes, any required or highly encouraged meetings, team meeting schedules
  • Other projects as directed by a manager
