About The Position

St. Mary’s Home Care and Hospice in Athens, GA, is seeking a PRN/Contingent Hospice RN Case Manager. They provide compassionate, exceptional care at home, utilizing an integrated virtual care program called Home Care Connect to help patients avoid preventable ER visits and hospitalizations. This program enhances clinical excellence with remote monitoring technology and 24/7 access to Virtual Care Center RNs. The RN Case Manager is responsible for professional nursing care to hospice clients, including assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes. They collaborate with physicians and the interdisciplinary team to manage patient symptoms, address family grief/loss issues, and implement the Hospice plan of care. The role also involves providing functional support to Hospice Aides and LPN/LVNs as needed. The position covers the Athens, GA territory and involves case managing a team of hospice patients, attending weekly Interdisciplinary meetings, and interacting with physicians as needed. The role is primarily Monday-Friday when needed, with occasional weekend coverage.

Requirements

  • Graduate of an approved nursing education program
  • Licensure as a Registered Nurse in the State of Georgia
  • One (1) year experience as a professional acute care nurse

Responsibilities

  • Provides professional nursing care to the organization’s hospice clients as prescribed by the physician, and/or requested by the client/family, and in compliance with the state’s Nurse Practice Act, any applicable licensure/certification requirements, and the organization’s policies and procedures.
  • Fully responsible for the client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes, is designated as the case manager for hospice patients.
  • In collaboration with the physician and other Hospice team members anticipates and manages patient’s symptoms, assists in identifying family/caregiver grief/loss issues and assists in implementing the Hospice plan of care.
  • Provides functional support to Hospice Aides and LPN/LVNs as needed.
  • Responsible for case managing a team of hospice patients.
  • Collaborates with interdisciplinary team and attend weekly Interdisciplinary meetings.
  • Interacts with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms.
