Requirements

• Registered Nurse with a Valid CT RN license.
• One year of RN experience.
• Valid driver's license and reliable transportation required.
• Computer proficiency required.

Nice To Haves

• Homecare or Hospice experience preferred.

Responsibilities

• Support patients, families, and loved ones through their end-of-life journey.
• Perform comprehensive assessments to establish goals for symptom management and end-of-life care using effective case management strategies.
• Act as a member of an interdisciplinary team to address patients' needs holistically.
• Develop and maintain therapeutic relationships through effective communication with patients and their families.
• Utilize state-of-the-art technology to document skilled services provided.
