About The Position

Residential Home Health and Hospice is hiring a Registered Nurse for a hospice position providing end-of-life care to patients in their homes in the Gardners, Pennsylvania area. The role involves working independently while collaborating with an interdisciplinary team to support patients’ comfort, dignity, and quality of life. The position is eligible for a $10,000 first-year success bonus tied to the accomplishment of key milestones. Field education, supportive leadership, and a collaborative culture are provided.

Requirements

Active Registered Nurse (RN) license in the state where the employee works.
Two years of RN experience and the clinical expertise to deliver quality patient care.
Valid driver’s license.
Ability to spend approximately 20% of the workday driving to and from patient locations.
Successful completion of a pre-employment drug screen.

Responsibilities

Deliver compassionate, patient-centered hospice care in collaboration with an interdisciplinary team.
Perform comprehensive patient assessments, manage symptoms and pain, and develop individualized end-of-life care plans.
Educate and support patients and families throughout the end-of-life journey.
Think critically and solve problems in real time.
Complete thorough, compliant electronic medical record documentation.
Travel to patient homes within the assigned territory.
