Requirements
- Experienced hospice or home health RN
- Passionate about providing exceptional care to patients in their homes, assisted living communities, or skilled nursing facilities
- Stellar communication and observation skills
- A deep understanding of the hospice philosophy
- A heart full of compassion and the dedication to truly make a difference
Responsibilities
- Provide high-quality bedside care to patients and their families
- Offer education on disease progression, end-of-life care, self-care techniques, and nutrition
- Collaborate with the interdisciplinary team to ensure comprehensive care
- Manage symptoms with expertise and empathy
- Document accurately, maintaining records of assessments, progress notes, care plans, and physician orders
