Hospice RN IPU Day Shift
About The Position
JourneyCare Hospice is seeking a registered nurse to join its hospice care team at the Barrington CareCenter in Barrington, Illinois. The role is responsible for patient and family care on the unit across the assessment, eligibility determination, admission processing, planning, implementation, and evaluation phases of the nursing process. The nurse coordinates with physicians, the interdisciplinary team, and other agencies to provide optimal care, reassesses patients and updates care plans, performs nursing procedures as directed, and supports families following a patient's death. The schedule consists of three 12-hour day shifts, 7:00 a.m. to 7:00 p.m., including every other weekend and three rotating holidays per year. The posting offers pay of up to $50.48 per hour.
Requirements
- Graduate of an accredited registered nursing program.
- Licensed as a registered nurse in the state of practice.
- Maintain current CPR certification.
- Ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public.
- Valid driver's license and proof of insurance.
Responsibilities
- Provide patient and family care on the unit, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation of the nursing process.
- Gather data and assess physical, psychological, social, and spiritual factors that may affect patient and family needs, and initiate interventions by other interdisciplinary team members.
- Communicate with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care.
- Conduct room visits to reassess patient needs and update the plan of care.
- Perform therapeutic, preventative, and rehabilitative nursing procedures as directed by the attending physician.
- Perform clinical pronouncement of death and provide support to the family.
- Maintain accurate patient records, charts, progress notes, and reports, and prepare care plans in a timely manner.
