About The Position

We are seeking a compassionate, driven, and highly organized Hospice Clinical Manager with strong leadership experience in hospice and/or home health care to help lead our clinical team and ensure exceptional patient-centered care. This role is ideal for an RN leader who understands the importance of compassionate end-of-life care, regulatory excellence, staff development, and operational efficiency. Your experience in managing clinical teams within hospice or home health settings will be instrumental in guiding quality outcomes and supporting patients and families through every stage of care. As a Hospice Clinical Manager, you will oversee daily clinical operations, mentor and support interdisciplinary staff, maintain compliance with state and federal regulations, and promote a culture centered on dignity, compassion, and excellence.

Requirements

  • Current Registered Nurse (RN) license
  • Valid Driver’s License
  • Current Auto Insurance
  • CPR Certification
  • Strong hospice and/or home health management experience

Nice To Haves

  • Minimum of 2 years of hospice management experience
  • Experience with HomeCare HomeBase (HCHB)

Responsibilities

  • Direct and coordinate hospice clinical services to ensure continuity, quality, and safety of care in compliance with state and federal regulations (Conditions of Participation)
  • Promote the agency’s philosophy, mission, and standards of compassionate patient care
  • Supervise and support clinical staff to ensure quality care delivery, appropriate assessments, and continuity of services
  • Participate in hiring, staff development, coaching, and ongoing clinical education
  • Collaborate with the Interdisciplinary Group (IDG) to develop and implement individualized patient care plans
  • Monitor and lead Quality Assurance and Performance Improvement (QAPI) initiatives
  • Ensure compliance with Medicare, Medicaid, and third-party payer requirements
  • Identify operational or clinical concerns and communicate recommendations to leadership
  • Promote infection control standards and patient safety practices
  • Foster strong communication, teamwork, and a positive clinical culture
  • Maintain continuing education and professional licensure requirements
