This position is Full Time working Monday-Friday 8am-5pm.

Requirements

  • Graduate from an accredited Diploma of Nursing Program or Associate’s Degree in Nursing.
  • Nursing skills as defined by generally accepted standards of practice.
  • Basic knowledge and skills to care for the clinical needs of the population served.
  • Professional and efficient verbal and written communication skills.
  • Good interpersonal skills.
  • Ability to multi-task, basic organizational and time management skills.
  • Basic computer skills.
  • Knowledge of general office and administrative practices and procedures.
  • Knowledge of how to utilize office equipment (copy machine, fax machine, etc.)
  • Ability to be open, sensitive, flexible and ecumenical.
  • Must have demonstrated experience in working with patients and caregivers dealing with life-threatening illness and end of life.
  • Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
  • Current Texas state license in appropriate discipline
  • Current American Heart Association CPR
  • Current Texas state driver’s license
  • Current auto insurance.
  • Must have reliable transportation.
  • Two years’ experience in healthcare setting in respective discipline

Responsibilities

  • Coordinates and directs the hospice patient’s care in their home environment (private residence, skilled nursing facility, assisted living, hospital, etc.) based on individual needs.
  • Independent management of the Hospice patient population requiring use of advanced assessment, teaching and decision-making skills.
  • Cares for a caseload of hospice patients by evaluating the patient for appropriateness of hospice care and developing the plan of care in collaboration with the provider, caregivers and Hospice IDG members.
  • Provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan.
  • Provides patient care based on provider’s orders, departmental policies and procedures, regulatory guidelines and best practice.
  • Provides care in collaboration with the provider, patient, caregivers, and Hospice IDG members to meet the goals of care with the consideration of the patient’s limitations and personal preferences in accordance with hospice philosophy.
  • Documents in the patient’s medical record in a thorough and timely manner according to agency requirements.
  • Attends and actively participates in IDG and team meetings.
  • Performs on-call responsibilities and Holiday staffing.
