As a Hospice Case Manager, you will play a vital role in providing compassionate, end-of-life care. Your ability to connect with patients and families while delivering expert clinical support will define their experience in their final journey. As an RN Case Manager your daily impact will involve: Conducting patient assessments while following the Hope CMS guidelines. Develop and implement individualized care plans. Manage patients' pain and symptoms. Educate families on the hospice process. Provide proper bereavement assessment during the patient pronouncement. Every day as a RN Case Manager will bring new challenges from navigating emotionally sensitive situations to balancing multiple patient needs. Success in this role requires emotional resilience, strong communication and proactive approach to patient care. You are provided autonomy to manage your schedule while being supported by a leadership team that prioritizes your well-being and the outcomes of the patient and family. It is important that the RN Case Manager live within a reasonable commute to manage a case load within the designated territory of the Lilburn/Stone Mountain or Lawrenceville area. This position has the expectations of working Monday - Friday, 8:00am - 5:00pm and attending required IDG meetings every other week in the office while managing a typical case load of 10-14 home and community-based hospice patients.

Requirements

  • Graduate from an accredited school of nursing.
  • Currently licensed as a registered nurse (RN) to practice in the state of Georgia.
  • Minimum of 2 years as a registered nurse.
  • Active patient contact within the past three years, preferred.
  • Ability to work independently.
  • Embody compassion and empathy for others.
  • Make accurate and at times quick judgements that involve managing pain and symptoms with precision.
  • Time Management & Organization with the ability to balance multiple patients without compromising care quality.
  • Ability to respond appropriately to crisis outside of a hospital setting.
  • Acceptance and adaptability to different social, racial, cultural and religious modes.
  • Have completed or the ability to complete a designated Hospice training program.

Responsibilities

  • Observe signs and symptoms and reports to the physician and other team members any unexpected changes in the patient's physical or emotional condition.
  • Teaches, supervises and counsels the Hospice patient and family members about providing care for the patient.
  • Organizes work schedule and utilizes time management to be able to attend all required meetings.
  • Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
  • Documents visit in electronic medical record system
  • Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor's orders.
  • Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient's physical and emotional condition.
  • Attends team conferences.
  • Coordinates the implementation of the plan of care for patients residing in SNF or ALF.
  • Works with interdisciplinary group concept of patient care.
  • Follows the policies and procedures of Hospice. Observes confidentiality and safeguards all patient-related information in compliance with the HIPAA regulations.
  • Complies with infection control policies and protocols.
