About The Position

Interim HealthCare is seeking compassionate, experienced Registered Nurses to serve as Hospice RN Case Managers. The role provides skilled, home-based nursing care to patients with terminal illnesses, offering autonomy to manage the workday and build one-on-one relationships with patients and families. The nurse collaborates with physicians and CNAs, supports patients and families through care planning, and coordinates care through discharge. The position is described as salaried, with weekly bonus opportunities.

Requirements

Active, unrestricted Registered Nurse (RN) license in the State of Michigan.
At least one year of clinical experience as a Registered Nurse.
Current CPR certification.
Reliable transportation, a valid state driver’s license, and current car insurance.

Nice To Haves

Hospice nursing experience is preferred.

Responsibilities

Provide home-based care to patients with terminal illnesses.
Work with physicians and oversee CNAs providing patient care.
Examine patients, perform assessments, document findings, and report changes to physicians.
Educate patients about their plan of care, prescribed medication, diet, and exercise.
Coach family members on the patient’s plan of care, medication, and home safety.
Ensure care goals are met and coordinate discharge from services.
