About The Position
HOSPICE RN CASE MANAGER New grads welcome! Are you a Registered Nurse (RN) looking to get into Hospice or an experienced Hospice RN looking for a change? We have partnered with a compassionate hospice care provider serving Philadelphia, Bucks, and Montgomery Counties. We're seeking an experienced Hospice RN or a motivated Home Care RN who is looking to gain knowledge and experience in Hospice. Autonomy and flexibility Willing to train No out-of-pocket costs for single-only benefits! JOB SUMMARY: Coordinate hospice care by working with other team members as well as with the patients/families to ensure needs are met. Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis. Perform evaluations for hospice care along with informational visits. Utilize the nursing process and best practices to provide professional care. Coordinate patient care services with other members of the care team as well as contracted services when needed. Update and document care plans and changes in the plan of care. Consult with the attending physician and Hospice Medical Director when needed. Actively participate in QAPI meetings and activities as well as staff development meetings.
Requirements
- Must be licensed RN in the state of Pennsylvania
- 1 year of hospice experience is preferred
- Must be a licensed driver with an insured, reliable vehicle
- Must have covid vaccinations

- 1 year of hospice experience is preferred
